Gordon House Application Forms

Gordon House provides specialist residential treatment to severely addicted gamblers in the UK.

Gambling Therapy is pleased to announce that the application form for access to Gordon House services is now available for download in .pdf format. The form (15 pages) will need to be printed off, completed and signed by hand and forwarded to the Referrals Team.

Once the form has been returned the Referrals Team will go through it to see whether they can offer accommodation and a service that meets your needs. If there are any other matters that they need to discuss with you, you will be contacted.
